The story picks up one year after the events of the original film. Kimberly Corman (A.J. Cook) has a horrific premonition of a massive pileup on Route 23, causing her to block the highway and save several people. However, just like in the first film, Death does not appreciate being cheated. The survivors soon realize that they are being hunted, and in this film, the deaths happen in reverse order of how they were supposed to die in the accident.
